Well I have never seen it (yet), but  that it is one of the respec items that drops in Nightmare or Hell difficulty. You collect 4 of them (each one is different), cube them, and you get an item that you can use to respec like you can at Akara after completing the Den of Evil quest.

o okay, on another thing how come i killed every monster in act 4 and i only got hell quest 3 and 4 nothing else no 1, 2&5...is the drop rate of them low or something?

Unfortunately these drops are only random based, there is no easy way to get all the pieces (exception for philos book which only spaw from the 2 superuniuques and the duriel clone).

Hephasto and Izual are supposed to be  more likely to drop them than other monsters.
